What is conscious sedation?

Intravenous sedation results in an altered state of consciousness and is required for all dental treatments. An anesthesiologist also performs anesthetic interventions for adults. It is very effective and popular. We can say that Globe Dental was among the first in Hungary to successfully introduce daily IV sedation in dental offices. We made it widely available to patients with dental phobia. We consider it important that we can perform dental treatment even in the case of fearful patients, if necessary under sedation, so that treatment postponed due to fear does not lead to a bigger problem. However, sedation does not mean deep anesthesia. In the case of dental treatments, it is sufficient in most cases (about 90%) to achieve painlessness (it is also more affordable than general anesthesia), but there are cases (about 10%) when deep anesthesia is required, which of course we also have the option for. Our anesthesiologist and dentist or oral surgeon colleagues can usually assess in advance whether sedation is sufficient for you or not.

What will happen?

1. Before starting treatment, the patient must always fill out a simple medical history form, which our anesthesiologists use to learn about the patient’s previous illnesses and any medications they may be taking.

2. Our anesthesiologist will then inform you in detail about the expected course of treatment and possible complications.

3. In order to maximize the safety of conscious sedation, our anesthesiologist will examine you and answer all your questions in detail. Once you have received all the necessary information, you will be asked to sign a consent form.

4. During the procedure, under the supervision of a specialist, the patient is continuously administered an anesthetic “cocktail” (such as propofol, dormicum, neodolpasse) intravenously, which, together with local dental anesthetics, ensures painlessness. During the procedure, the dentist and dental assistant focus on the dental treatment, while the anesthesiologist continuously monitors the patient, checking their heart function, pulse, blood pressure, and blood oxygen levels.

5. After the procedure, there is a 30-60 minute observation period, after which the patient is discharged to their home with their companion, along with important information about post-sedation care.

Deep anesthesia - for adults

Deep anesthesia is nothing more than when, in addition to securing the airway, the patient is unconscious, does not feel pain and does not remember the surgery, but shortly after the end of the procedure, consciousness returns, and they are able to swallow, speak, and move. The anesthesiologist ensures close monitoring of the patient. There are patients who, for some reason, require deep anesthesia to complete their treatment.

There may be several reasons for this:

  1. Some people have suffered some kind of trauma during their dental treatment and have been terrified of the word dentist ever since. Some patients suffer from pathological nausea or uncontrollable retching and are unable to tolerate dental treatment. It also happens that someone wants to have several procedures performed in one session, the number of which can be tripled under anesthesia, so busy businessmen often decide in favor of this type of procedure.
  2. Anesthesia always takes place under monitored conditions: an ECG monitor, blood pressure monitor, and pulse oximeter measuring blood oxygen level are placed on the patient. After this, anesthesia can begin, the first part of which is the anesthesia phase. This starts with venous insurance, then continues with the administration of medications through a venous cannula. The drugs cause patients to fall asleep within a few seconds.
  3. In the case of deep anesthesia, the airway is secured and ventilation is started after the patient falls asleep. There are several ways to do this. During the surgery, anesthetic and pain medications are continuously administered, continuous ventilation is maintained, and adequate fluid intake is ensured. The anesthesiologist is responsible for managing any possible anesthetic complications.
  4. At the end of the surgery, the anesthetic medications are discontinued, and the patient begins to wake up as his or her body clears these medications. The airway devices are removed during the awakening phase, when the patient is still practically asleep, but spontaneous breathing has returned. After that, they wait for full awakening, which takes approximately 10-15 minutes.
  5. In all cases, deep anesthesia is performed by an anesthesiologist with modern technical conditions. Before the treatment, an medical examination is required, which includes an ECG and a laboratory test (blood count to check liver and kidney functions, blood clotting factors and serum ion levels). Eating, drinking and smoking are strictly prohibited 6 hours before anesthesia treatments! In the event of any cold, fever or flu-like symptoms, the treatment must be postponed, as a completely healthy state of health is required for deep anesthesia.
  6. After interventions within 2 hours, several hours may be required, and after longer treatments, 24-hour observation may be required.

What you need to know after deep sedation:

With careful application, risks can be minimized and side effects can be managed. However, even with the greatest care, unforeseen consequences can occur:
  • The most common complication of anesthesia is dry throat, a scratchy feeling in the throat. We protect against this with special gargling liquid and oral fluid therapy.
  • In certain surgeries, nausea and vomiting can be expected after the operation. To prevent this, we give the patient antiemetic medication before and during the surgery to avoid discomfort.
  • Feelings and chills are also common, which we try to protect against by warming your body during the surgery, giving an infusion heated to body temperature, and using warm bedding to provide comfort after the surgery.
  • Our patients who have undergone anesthesia often complain of headaches and confusion. However, this is temporary and can be prevented with appropriate medications.

Our best advice before deep sedation:

Of course, you can do a lot to ensure that your dental procedure under general anesthesia is free of side effects. We recommend that you:
  • Always bring your regular medications and medicinal products with you.
  • If you smoke, try to avoid cigarettes two days before the surgery, this can greatly reduce respiratory complications.
  • If you want to minimize the harmful effects of alcohol, stop drinking alcohol 3-4 weeks before the surgery, as this will most likely avoid severe headaches and confusion.
  • If you feel unwell, have a headache or sore throat before the surgery, be sure to inform the specialist performing the surgery, as it may affect the outcome of the surgery.

What happens before sedation? During conscious sedation, in order to ensure maximum safety, we ask that you refrain from eating or drinking anything for 4 hours prior to the procedure. This is because sedation reduces your level of consciousness, making it difficult to control swallowing and coughing. Therefore:
– Do not consume any food or liquids for 4 hours prior to sedation!
– Take your regular medications (e.g., blood pressure medication, medication for vascular and heart disease, etc.) at the usual time!

Following sedation and dental treatment, we monitor the patient for a personalized period of time. We do not recommend driving after sedation. We ask patients to leave our dental clinic accompanied by someone else and not to make any further plans on the day of the procedure, but to rest at home.
Following intravenous sedation, the medication does not leave the body immediately, so we monitor the patient for at least 30 minutes after the dental treatment. The patient may leave accompanied by a responsible adult. If you arrive by car, you may not drive.
– For the rest of the day, you must not engage in any potentially dangerous activities (e.g., driving) or consume alcohol. It is recommended that you refrain from strenuous physical activity for a few days.

Patients in good health with controlled high blood pressure are suitable for conscious sedation without further ado; the anesthesiologist will assess their health before the procedure. In the case of any factor affecting health, acute or chronic illness, the anesthesiologist will decide on the suitability of sedation. We do not perform conscious sedation on children or pregnant patients.

The advantage of conscious sedation over deep anesthesia is that it does not cause nausea, headaches, or dizziness. A rare side effect may be a slight feeling of numbness during the procedure. Conscious sedation has an inhibitory effect, and patients with a cheerful disposition often laugh. In our practice, after performing hundreds of conscious sedations, patients have recovered without complications.

Of course, there are contraindications, which the anesthesiologist will inform you about after analyzing your medical history and test results (laboratory, ECG).

The fee for deep anesthesia depends on how many hours of anesthesia are required. The fee for the first hour is 130,000 HUF, and the fee for each additional hour ranges from 55,000 HUF to 75,000 HUF, depending on how many hours are required.
